TEXT MESSAGING TERMS AND CONDITIONS

These Text Message Program Terms and Conditions (“Terms”) govern participation in the Patient Text Messaging Program for CDI Management Corp. d/b/a RAYUS Radiology and Center for Diagnostic Imaging, Inc. d/b/a RAYUS Radiology (collectively “RAYUS”) and their affiliated and/or managed entities.

  1. PROGRAM NAME

RAYUS Radiology Patient Text Messaging Program

  1. PROGRAM DESCRIPTION

The Program provides patients with text message communications regarding appointments, scheduling and rescheduling, exam preparation instructions, registration reminders, imaging procedure notifications, prior authorization updates, billing and payment reminders, follow-up care communications, patient satisfaction surveys, patient portal notifications, service updates, and other healthcare-related communications.

  1. CONSENT TO RECEIVE TEXT MESSAGES

By opting into the Program, you authorize RAYUS Radiology and its service providers to send automated and non-automated text messages to the mobile telephone number you provide. Consent is not a condition of receiving treatment or services.

  1. MESSAGE FREQUENCY

Message frequency varies. Patients may receive recurring messages depending on appointment activity, imaging services scheduled, account activity, and communication preferences.

  1. MESSAGE AND DATA RATES

Message and data rates may apply. Wireless carrier charges may apply.

  1. HIPAA, PRIVACY POLICY, AND NOTICE OF PRIVACY PRACTICES

RAYUS Radiology is committed to protecting personal information and PHI in accordance with HIPAA and applicable law. Text messaging is not always secure. The Text Messaging Privacy Policy and Notice of Privacy Practices are incorporated into these Terms.

  1. CONSENT ACKNOWLEDGMENT

By enrolling, you acknowledge that message frequency varies, message and data rates may apply, you may opt out by replying STOP, and obtain assistance by replying HELP.
